Information Provided by Customers

  1. Individuals are required to provide the following information at the time they register with us: first and last name, preferred salutation, company name, email address, telephone number, email preferences, user name, password, and answers to two challenge questions.
  2. All authorized administrators in an organization are required to provide the following information at the time they create an Administrator Profile with us: first and last name, email address, telephone number, email preferences, user name, password, and answers to two challenge questions.
  3. Administrators may be required to provide certain types of information if they wish to be granted or assigned certain privileges on our website.
